Finding and accessing needed health care services can be stressful and overwhelming when you or a loved one require immediate care, or are making ongoing decisions.
To improve overall efficiency
and positive patient interactions with regional health services across the province, the Ontario Government is building a connected health care system centred around patients, families and caregivers. These Ontario Health Teams (OHTs), are groups of health-care providers and organizations that are clinically and financially accountable for delivering a full and coordinated continuum of care to their specific geographic population.
Before the creation of OHTs, Primary Care, Acute Care, Mental Health and Addictions Services, Long-Term Care, Home Care and Community Care all had to be sought out individually by the patient.
Now, through Ontario Health Teams, you as a patient can receive all your care from one team, regardless of what stage of health care you’re currently in.
